Agreements and Terms

All customers who order replacement contact lenses from
SmartViewContacts.com must not have an expired prescription (i.e. it must be
less than 2 years old).

| ii |
We are not responsible for customers' discomfort or lack of suitability to wear contact lenses. It is your responsibility to ensure that the contact lenses you order is exact to the type that you are prescribed for. |

We rely on you to supply us your latest prescription information provided by your optician or optometrist through the online form, fax, or verbally over the phone. |

| iv |
We assume that no eye care professional has ever advised you that you are
not suitable to wear contact lenses, or you should stop wearing the
prescribed contact lenses. We also assume that customers have not suffered
from any eye condition requiring treatment within the last 12 months. |

We retain the right to stop supplying contact lenses in the event that the selected lenses may put you at risk of any kind, or are not in conformity with the specifications provided by your eye care professionals. |

You agree to pay by the method indicated on your order form and agree to
accept and pay for the shipment method that you choose for the selected
products by completing the online order form. |

You agree that you are the authorized owner of the credit card and accept responsibility for all charges made on SmartViewContacts.com by your card by completing the online order form. |

SmartView Contacts is only a replacement contact lens company. You can only
purchase from us the contact lenses that have been prescribed by your eye
care professional and that you are wearing successfully. Please confirm that
you are successfully wearing the selected lenses for which you have a valid
prescription before proceeding with your order. |
